Quick answer — Transformer overheating is most often due to overloading beyond the design thermal limit, a loss or failure of cooling (fan or pump, blocked radiators, low oil or a faulty pump), high ambient temperature, high ambient or low oil level, or poor oil flow from a sticky valve or sludge. Once the hottest-spot temperature rises too far, the insulation ages rapidly and can fail.

What causes transformer overheating?

Load and cooling

Running above nameplate without the corresponding cooling (ONAN/ONAF/OFAF) raises the winding and oil temperature. A failed fan or pump, blocked radiator, or a low oil level removes the designed cooling.

System and environment

High ambient temperature derating, convector blockage, and internal sludge or poor oil flow all reduce the effective cooling or raise the loss.

Consequence

Each extra 6-10 K above the design hottest-spot temperature roughly halves the insulation life, and prolonged overheating can release moisture and lead to a thermal or electrical failure.
