The cause

A severe internal fault (arcing, a short) generates gas and pressure rapidly; if the relief is blocked or too slow, or the pressure exceeds the tank wall strength, the tank can rupture.

Why a burst

A weak or corroded tank, an oversized/failed relief, or a protection failure all raise the rupture risk.

Prevention

Correct protection (differential, Buchholz, pressure-relief), a functional pressure-relief device, a sound tank, and a good condition/no-overload practice minimise the risk.
